Who is eligible for a health check?

The health checks are aimed at those aged 45 and over who have not already been told they have heart disease or diabetes by their GP.

There are two main ways to get your health check:

1. Make an appointment at your GP surgery. All GPs in Bolton are signed up to this programme and will be ready for your call. They will also be writing to patients they haven't heard from recently to invite them in for their check.

If you are registered with a GP outside the Bolton area, you will need to ask your surgery if they are offering the health checks.

2. Attend a community health check. Our qualified and experienced NHS Health Trainers will be offering free health checks at different venues around the Borough.

If you attend a community health check and are registered with a Bolton GP, the results of the check will be sent to your GP with your agreement.
